Please be aware that by joining "Team LGS Foundation" for the Walk for Epilepsy, you are not
officially registered with the Epilepsy Foundation of America and will not receive a t-shirt from the
Epilepsy Foundation of America. You are agreeing to participate in the “LGS Foundation” group
and are submitting your information to the LGS Foundation for their records. You will also receive
a t-shirt from the LGS Foundation for the walk. It is highly recommended that you register on the
National Walk for Epilepsy website (www.walkforepilepsy.org) and agree to their terms and
conditions, as they are the hosts of the event. The LGS Foundation is not the host of the
National Walk for Epilepsy. The National Walk for Epilepsy will remain property of the Epilepsy
Foundation of America. In addition, you will be expressly assuming the risk and legal liability and
waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward
might sustain as a result of participating in any and all activities connected with joining team
“LGS Foundation.”
By submitting your registration, you recognize and acknowledge that there are certain risks of
physical injury to participants in this event, and voluntarily agree to assume the full risk of any
injuries, damages or loss, regardless of severity that your minor child/ward or you may sustain
as a result of participating in any and all activities connected with or associated with this activity
and are encouraged to seek a physician’s approval. You further agree to waive and relinquish all
claims against the LGS Foundation for you and your child. You will not hold LGS Foundation or
the Epilepsy Foundation of America, including officials, agents, volunteers and employees, any
of the event’s sponsors, promoters and all other persons or organizations associated with the
LGS Foundation or the National Walk for Epilepsy responsible for any and all claims for injuries,
damages or loss that your minor child/ward or you may have.
By submitting your registration, you consent that your photographs, voice and likeness may be
used by the LGS Foundation.
